(1) Event in first stage of labour:-
(i) Fundal dominance in uterine contraction:-
Contraction stasts fundus
Pass across & down ward
Help in cervical dilation
(ii) Progressive Uterine contraction & Retraction:-
The Longitudinal muscle fiber of the upper uterine segment attach with circular muscle fiber of the lower segment.
With each uterine contraction, not only the canal is opened up from above down but it become shortened & retracted.
These are co-ordination b/w fundal contraction and cervical dilation
Upper segments contract the cervix dilate & pushes the fetus to the lower segment
Upper uterine segment is active
Lower uterine segment are passive
(iii) Cervical Effacement:-
Infusion of cervical canal in to lower uterine segment
Muscle fibers surrounding internal as draws upwards & merges in to lower uterine segment.
In primigravida effacement occur primarily & than dilator
In multi both occur simultaneously
(iv) Cervical Dilation:-
It is process of enlargement of the as to an opening large enough to permit passage of fetal head.
Dilatation occurs due to uterine contraction & Retraction & pressure applied by presenting part & bags of water.
(v) Formation of bags of water (membranes):-
The membranes are attached loosely to the presentation the girdle of contact of the head.
Due to shortening of lower uterine segment membrane get detached increase intra uterine pressure.
Due to this membranes bulges down word forming bag of membrane.
When presenting part is engaged amniotic fluid is divided in to two parts
Former is form water
Later in hind water
Due to stretching of lower uterine pressure causes the membrane to bulge downward bag of membrane or water.
(vi) Fetal Excess pressure:-
Contractions are transmitted to upper pole of fetus & then down the long axis of fetus & apply by presenting part of cervix
Help in cervical dilation
(2) Event’s of second stage of labour:-
This stage concerned with the
(i) Continuous descend of the fetus
(ii) Delivery of the fetus-
Two fetus work in this stage
Expulsive force of uterine contraction
Bearing down effort (Voluntary contraction of abdominal muscle)
Lady holds her birth
Starts with full dilation of cervix
(3) Events of third stage of labour:-
(1) Placental Separation:-
The birth of the baby there is marked retraction which recduces the surface area of placental site effectively.
But placenta is inelastic so it can’t keep race with such as extent of diminution resulting in its buckling.
Two ways of separation of placenta are-
Central (Sedulity machine)
Marginal (Deccan Mech.)
(2) Expulsion of placenta:-
After complete separation it is forced down ward in to upper part of vagina by contraction & retraction of uterus.
Then expelled out by bearing down efforts.
Either way the teacher or student will get the solution to the problem within 24 hours.
